Agreements, in the open.
A practice accepts two agreements at registration: the Master Service Agreement (which incorporates the Terms of Service, the Service Level Agreement and the AI acknowledgment) and, with a typed signature, the Business Associate Agreement. The Privacy Policy is made available and acknowledged. Each version is dated and fingerprinted; when a document changes, practices are asked to accept the new version before continuing, and executed copies can be downloaded from the app.
Master Service Agreement
The subscription agreement between the practice and Nuvanei Health. Incorporates the Terms of Service, the Service Level Agreement and the AI acknowledgment as exhibits.
Business Associate Agreement
Establishes Nuvanei Health's obligations as your Business Associate under HIPAA for the protected health information it processes on behalf of your practice. Executed separately with a typed signature.
Privacy Policy
How Nuvanei Health collects, uses, shares and protects personal information about practice users, patients and visitors. Made available and acknowledged; no signature.
Terms of Service
Exhibit A to the Master Service Agreement: the terms of use that apply to every individual who accesses the platform.
Service Level Agreement
Exhibit B to the Master Service Agreement: the 99.9% availability commitment, maintenance windows, support response targets and service credits.
AI & Private LLM Acknowledgment
Exhibit C to the Master Service Agreement: how the AI-assisted functions work on private model deployments, what they may never do, and each user's review responsibilities.
Documents patients accept in the app.
Patients are invited by their practice and accept these in the patient app. They are written for patients, and patients never see a practice's business contracts.
Patient Terms of Use
The rules for using the patient app: who may use it, your account, what the app does and does not do, acceptable use, notifications, payments and your content.
AI Disclosure & Consent
Explains how assistive software is used in your care, what it is never allowed to do, how your information is protected, the risks and limitations, and your choices including human-only communication and withdrawing consent.
Patient Privacy Notice
How your health information is protected when your practice uses the app, what the app collects, and where to direct questions. Made available and acknowledged; no signature.
Telehealth Consent
Informed consent to receive care by video or telephone, including the benefits, risks and limitations of telehealth and your rights as a patient.
